Whenever we can we are always pleased to help our clients. This document gives details of our terms and conditions of service. If, however, you need clarification or have any queries, please contact us and a member of staff will be happy to help you.

Please be aware that any information provided through any part of our web site (examples: forum, blog, news, chief site, live chat, phone, e-mail, ) is for fun & amusement purposes only and does not represent professional advice. Dream Smile Dental Clinic does not have a contract with a NHS Primary Care Trust and all treatments are provided private only.

Treatment planning:

Once your treatment plan has been agreed with the Dentist, we'll supply printed information on your plan. Whether this plan alters because of clinical or radiographic findings, we'll advise you and discuss this with you. Treatment plans are not invalid for net 90 days from your date. If there are any points on your own treatment plan that you want to query, please do not hesitate to ask us, before the appointments for actual treatments are made.

Authorization forms:

Certain treatments require completion of a written consent form. This is so that you can explain the treatment, aftercare and any risk to you thoroughly, before these treatments are carried out.

Fees:

Dream Smile Dental Clinic doesn't manage a credit account system and we require fees to be settled at the appointment.
Where a laboratory fee is incurred by treatment, at least 50% of the total fee is due at the appointment where impressions are taken. Fees for particular treatments like Invisalign and Dental Implants are taken at each visit in payments that were staged.

Where treatment is paid for on finance through a Finance company, please be aware the finance agreement must be accepted and signed prior to the start of that treatment. Take note that finance is only accessible on treatments finished?400, not including Emergency Dental Treatment, and a minimum deposit may also be needed.

Pearl Dental Clinic does not accept any payments.

Fees for treatment where intravenous sedation is comprised, must be settled prior to the appointment to avoid fiscal trades, or signatures being needed, whilst a client continues to be under the influence of the sedative.

Dream Smile Dental Clinic reserves the right to charge time based deposits for booking future appointments. Deposits are deductible off the cost of treatment.

Dream Smile Dental Clinic reserves the right to make a charge for any debt passed to your debt collection agency.

All the fees employed for any form of Dental treatment at the practice are based on; the stuff used, laboratory costs incurred ( in case that it's appropriate), and the clinical time spent. The fees are however, illustrated as treatments prescribed on the treatment plan for your information.

Late cancellation or missed appointments:
Dream Smile Dental Clinic reserves the right to bill up to?100 per hour, in the function of an appointment canceled with less than 24 hours notice or a missed appointment.

We realize that some patients travel long distances to get to the practice, and in some events being late for appointments can be ineluctable. If you are more than 10 minutes, please be aware that you might be requested to reschedule your appointment.

Guarantee:

Ceramic Veneers, at Dream Smile Dental Clinic our Lumineers, Implant crowns and Ceramic crowns are ensured for a period of 5 years following placement, provided that the following conditions are met:

The treatment has been completely paid for by the patient and does not owe the practice any cash for the treatment received.
Implant the Veneers, Lumineers, Crowns & crowns haven't been damaged as a result of a collision, excessive grinding or injury.
The patient has followed all post treatment care recommendations.
The patient has attended and routine assessment every 6 months.
Private Details:

It really is crucial that you give the full medical history and details of any medication you take. Should these change in any way, it is very important for you yourself to tell your Dentist. It is the patient's obligation to notify the practice of any changes in either their medical history personal details and/or.

Use of Pictures and X rays:

Dream Smile Dental Clinic may use images and x-rays of your smile and teeth just, for promotion and educational purposes on the Dream Smile Dental Clinic site, and on educational and promotional literature. Your name will never be published, and identity WOn't ever be disclosed. However, if you DO NOT want for us to use your images and x-rays this way, please let us know.

Use of patient contact details:

At Dream Smile Dental Clinic the health of our patients is our highest priority, and we also like to keep our patients of our latest special offers and informed of various changes that are important at the practice. We like to remind our patients in their appointments, when they're due for appointments, and other various important reminders. On this particular note, you might be occasionally contacted by the practice via phone, text, email or by letter in the post. If you DO NOT want to be contacted by the practice by any or all of these means, please let's know.

Grievance's policy:

To be able to ensure that every patient has just the very best experience whatsoever times, at Dream Smile Dental Clinic we consistently take complaints about any aspects of our services very seriously.

Criticisms should be made to the Dream Smile Dental Clinic Management', and should be clear, so that they can be dealt with efficiently.

Every complaint will receive immediate written/e-mail recognition, and the Management will strive to resolve the complaint within a fast, decent time period (usually about 2 weeks). For our full complaints process and policy, please request a member of our reception team. If a complaint is about any aspect of clinical care or affiliated charges it then referred to the treating Dentist and will generally be acknowledged by the Management team. Take note that in these events, Dream Smile Dental Clinic acts as a liaison between the patient and the treating Dentist, merely and accepts no liability for the treating Dentist.

If still unsatisfied, you may want to forward your complaint to the General Dental Council.

No endurance/Mistreatment policy

In these scenarios, the right to refuse treatment and entrance is reserved by Dream Smile Dental Clinic.

Data Protection Act:

We store all patient personal details on a secure computer system relative to the Data Protection Act. All clinical notes, digital radio-graphs digital photos etc, remain the property of Dream Smile Dental Clinic. Copies of notes, radiographs and photographs can be made accessible on request, and Dream Smile Dental Clinic reserves the right to charge an administration fee.
